The web page was created in a text editor,
saved as "text," and named "index.html."

Text editors I have used:
  • Word 4 (later versions are difficult)
  • SimpleText (works great)
  • TextEdit (works ok, but is a pain*)
*You can save HTML files using TextEdit.
Choose "Make Plain Text" from the Format menu before you try to save the file.
When you save, specify ".html" as the filename suffix and choose "Don't Append" when TextEdit asks if you want to add the .txt extension.
OR
In TextEdit's Preferences window select "Plain text" instead of "Rich text," then select "Ignore rich text commands in HTML files."

My favorite text editor is (now $200) BBEdit.
